The standard Heart Failure Zone status worksheets as an interactive app.
This application provides interactive check lists of each of the three categories Green(Good/The Goal), Yellow(Caution), and Red(Danger), and displays your status on the main screen. If you should consult with your health care provider or need immediate medical assistance, the color of the zone, yellow or red, is displayed on the status screen and the name and phone number are shown with a one click call to that number.
The phone book tab provides an interactive screen to provide the name and phone number of your heart clinic or doctor. Similarily, there is a field for the emergency contact that defaults to 911. You can also select the Browse button that will navigate to the phone Contacts.
Each of the Green, Yellow, and Red heart tabs provide a checklist for the zone and will set the status shown on the main screen accordingly. If any Red Zone item is selected, the zone will be Red. If all Red remain unselected and at least one Yellow Zone item is selected, the status will be Yellow. If all Red and Yellow Zone items are unslected, the status will be Green by default without selecting any of the Green items
Living with and managing congestive heart failure requires constant awareness and daily monitoring of weight, and the condition of your body, such as swelling of the ankles or abdomen. Frequently reviewing check sheets of symptoms and identifying if your health is improving or beginning to show deterioration will allow you to be proactive in working with your health care providers. This application provides an interactive tool for these lists of symptoms and allows you to easily store and one click dialing of appropriate medical services.

Green Zone Means that you have met your daily health Goal.
• Your symptoms are under control
• Continue taking your medications as ordered
• Continue daily weights
• Follow low-salt diet
• Keep all physician appointments
Yellow Zone Means Caution. Your symptoms may indicate that you need an adjustment of your medications. Call your physician, nurse coordinator, or home health nurse.
• Weight gain of 3 or more pounds
• Increased cough
• Increased swelling
• Increase in shortness of breath with activity
• Increase in the number of pillows needed
• Anything else unusual that bothers you
Red Zone Means that you need to be evaluated by a physician right away.
• Unrelieved shortness of breath
• Shortness of breath at rest
• Unrelieved chest pain
• Wheezing or chest tightness at rest
• Need to sit in chair to sleep
• Weight gain or loss of more than 5 pounds
